Name, email address, phone number (optional), the message you write, and whether you identified yourself as a physician or prescriber, or as a patient or caregiver.

Neither form is intended to collect sensitive personal information as defined by the Act. Both carry a notice not to send health information, and the pharmacy asks that you do not. If health information is sent anyway, it is deleted from the enquiry record once the enquiry is closed.
Contact form data is used to answer your enquiry and, where you are a prescriber, to set up or maintain a professional account with the pharmacy. Launch notice data is used to send a single message when the online services open.
The lawful criterion relied on is your consent, given when you submit the form, and the pharmacy's legitimate interest in responding to professional enquiries.
Personal data collected through this website is not used for advertising, profiling, or automated decision-making, and it is not sold.
Data is kept only as long as needed for the purpose it was collected for, then disposed of securely. Confirmed periods are published here as they are set.

Right to data portability
To obtain data you provided in an electronic format, where the Act applies.
Right to damages and to complain
To be indemnified for damage arising from unlawful processing, and to lodge a complaint with the National Privacy Commission. Raise it with the pharmacy first if you can — it is usually faster.
